This position is a case manager providing home based services to clients with severe and persistent mental illness. The case manager functions as an integral part of the multi-disciplinary, PACT treatment team. The individual caseload will be from 12 to 18 clients. Job duties include but are not limited to: developing/maintaining recovery oriented treatment plans, leading individual and group psychosocial rehabilitation, providing case management by linking consumer to other community resources, and providing crisis diversion when necessary.
